ATHLETIC PARTICIPATION REQUIREMENTS
The following items need to be turned into the MAIN OFFICE before students participate in Athletics.

Interscholastic Participant Form:
Remember to fill out back and second
sheet .

Emergency form: One of these needs to be filled out for every sport.

Fee: $50 per sport up to a maximum of $125. Checks made out to Cresthill Middle School.

Respect pledge: Only one needs to be filled out per year.
All of the above must be turned in before your student can participate in practices and games
