Registration Documents 2011 / 2012 All Forms are Updated and Available!
Each of these forms need to be downloaded, printed and completed. Please return all completed forms to the Registrar. If you need to FAX your completed forms to the office, please FAX them to (303) 387-5001. Please call the Registrar (303) 387-5004 to verify that all forms sent by FAX have been received, or if you have questions concerning any of the forms. Don't forget that you will need to submit a copy of a certified Birth Certificate, Immunization Records, and Proof of Residency with your enrollment package.
Kindergarten Minimum Immunization Requirements Document The 2011 /2012 Minimum Immunization Requirements document details the minimum immunizations your child must have prior to beginning Day 1 of Kindergarten.
Kindergarten Initial Session Request Form The initial Session Request form is used to inform the school on session assignments that work best for you. While no promise of a particular session can ever be guarenteed, it allows the Principal to consider your preferences when making your childs session assignment. This form is one page.
Kindergarten Full Day Acceptance/Tuition Schedule This form is to be signed by parent/guardian after student is accepted into the Full Day Kindergarten program.
Enrollment Packet Cover Letter This cover letter doubles as a check list to ensure you have returned all items required to complete the student registration process. This form is two pages.
2011/2012 Registration Form This is the Douglas County School District Registration Form. All sections of the form need to be completed and parent signatures are required on all pages. This form is four pages.
Records Release Form Authorization to release student information is completed for any student that attended another school prior to enrolling at Castle Rock Elementary. This form is one page.
School Bus Contract Douglas Country School District Bus Contract. ALL students must have a bus contract on file, even if they do not ride the bus on a regular basis. This form is one page.
Home Language Survey Federal and State regulations require schools to determine the language(s) spoken and understood by each student. This is in accordance with the English Language Proficiency Act of Colorado. This form is available in muliple languages, please let the school know if you need this form in a different language. Field Trip Permission Form This particular Field Trip Permission Form is maintained by the teacher in your child's classroom. Completion of this form is NOT giving permission for any field trip during the year, you will receive individual permission slips from your child's teacher for these. This form is one page
Emergency Form The Emergency Form is maintained by the teacher in your child's classroom. This important information is used when school is dismissed early because of weather or other kind of emergency. Please keep the school informed of changes to your emergency plan and ensure your child understands what he/she will be doing. This form is one page.
District Information Technology Agreement Every student must have this signed agreement on file inorder to be able to use the schools computer and internet assets. This form is one page.
Parent/Guardian Media Consent Form Parental consent must be obtained before we can publish your childs image and/or school work. Please review this form carefully, answer questions 1 - 4 and sign it. This form is one page.
Migrant Education Program Completion of this form is mandated by Colorado Law and the Colorado Department of Education. All information you give us is strictly confidential. This form is one page.
Special Service Team Enrollment Form This form is used to inform the Special Service Team of any services your child will need that is outside the normal classroom setting. This form is two pages.
Student Residency Questionnaire The Student Residency Questionnaire is intended to address the McKinney-Vento Act. Your answers will help the administrator determine residency documents necessary for enrollment of your child. This form is one page.
Health Information Form This form is required for all new registrations in the school. This information is reviewed by the district nurse who in turn will annotate health concerns into the student record. This form is three pages.
