Below are the forms necessary to enroll a child at Frontier Valley.

Please pay close attention to the school year because the forms differ.

Kindergarten Registration
2010-2011

Children must be 5 years old on or before October 1, 2010, to enter Kindergarten.

Wednesday, January 13, 2010   4:00 pm - 6:00 pm
Thursday, January 14, 2010 8:00 am - 4:00 pm

Enrollment packets will be available for download on the Frontier Valley website January 4, 2010. Packets will also be available in the main office effective January 4, 2010.  The following documents must be submitted with the completed enrollment packet:  Birth Certificate, Immunization Records and Proof of Residency (warranty deed, property tax statement, deed of trust or lease agreement).
KINDERGARTEN PACKETS MAY NOT BE TURNED IN BEFORE
JANUARY 13TH  AT 4:00 PM!

Families that do not currently attend Frontier Valley and wish to enroll a child in grades 1-6 for the 2010-2011 school year, may submit an enrollment packet and required documentation anytime after January 4, 2010.  Packets will be date stamped and processed in the order of receipt later in the spring.
