KODIAK FIELD TRIP TO INTERNATIONAL TOWNE (March 19th & 20th)

1.   Please click on the link below for Revtrak and click on Registrations.

2.   Then, select "Field Trips."

3.   Then, click on the "2012/13 MMS Kodiak IT FT"."

4.   Then click on "CLICK HERE to Register for this Class."

a.   Create an account (email address and password) if you have not already done so (some people may have used this system if paying for athletic or club fees).

b.   If this is the first time you have used this system, you will then fill out your information (you will not need to do this in future registrations).

c. Once entered, if your child’s name is not displayed, you will need to click the "Add Participant" button to add your child (again, a 1 time only thing).

5.   Then, select the radio button in front of your child's name before you fill out the form.

6.   Complete the registration and payment information (even if there is no fee).

7.   You should receive a receipt via email once registered.  If you do not get a receipt within 90 minutes, you did not complete the process and your child will not be registered for the field trip.

If you have problems registering and completing our payment online, please contact our school bookkeeper, Sylvia Brockman, by emailing her at: sylvia.brockman@dcsdk12.org or calling her at 303-387-4759 (7 am – 3:30 pm).

If you do not have access to a computer, you may pick up a paper copy of the registration/permission slip in the Mesa Middle School Main Office with Sylvia Brockman and submit the hard copy to her by 3/11/2013.
